All patients must bring with them to their appointment a valid PA ID as well as any relevant medical records that document the specific medical condition you have that would make you eligible. Please see Qualifying Conditions for a list of medical conditions approved for medicinal cannabis use.
Patients that want to participate in the medical cannabis program must follow these steps:
First, visit the Patients and Caregivers Registry and create a patient profile in the Department of Health’s patient and caregiver registry.
Next, obtain a physician’s certification that you suffer from one of the 21 serious medical conditions.
Then, return to the Patient and Caregivers Registry and pay for a medical marijuana ID card.
Obtain medical cannabis from an approved dispensary in Pennsylvania.
The Pennsylvania Safe Harbor Letter is available to parents, legal guardians, caregivers and spouses of a minor under the age of 18 who suffers from one of the serious medical conditions defined in the Medical Marijuana Act.
Prior to beginning the Safe Harbor Letter application process, please make sure you have all of the required documents and information necessary to submit your application. All applicants will need:
Completed electronic copy of the Safe Harbor Physician form.
Completed electronic copy of Pennsylvania background check.
Legal guardians will need an electronic copy of their guardianship papers.
Caregivers will need an electronic copy of their caregiver status.
Spouses will need an electronic copy of their marriage certificate.
Once a certification has been established, it's highly recommended that each patient keep a log of their usage to better help your doctor track effectiveness of the prescribed dosage and subsequent positive or negative impact on a patient's medical condition.
